Core Tip The short- and long-term outcomes of PCI in CBLs are closely related to the procedural success and optimization of the bifurcation stenting technique. Failure of stent implantation at the appropriate site of the side branch of a CBL or leaving excessive stent metal at the bifurcation site are important risk factors for complications. With the help of a novel guide extension assisted stenting technique by using a GuideLiner mounted on both guidewires in the branches of the bifurcation lesion and advanced to the carina of the bifurcation, a stent can be implanted at the most possible appropriate site of the side branch in side-branch mono-ostial (Medina 0,0,1) or in the distal mono-ostial (Medina 0,1,0) in non-true CBLs. The technique can also be used in two stent techniques for complex true CBLs (tri-ostial or Medina 1,1,1). The technique is described in full using 3 coronary bifurcation cases and even demonstrated schematically in the first case.
