Core Tip	Depression is a common and impactful comorbidity in gastric cancer patients, significantly affecting treatment adherence, quality of life, and survival. A bidirectional relationship exists between depression and gastric cancer, where psychological distress exacerbates cancer progression, and cancer-related burdens worsen depressive symptoms. Effective management requires a holistic, integrated approach, combining pharmacological treatments, psychotherapeutic interventions, and complementary therapies. Multidisciplinary care, incorporating emotional support, pain management, and nutritional strategies, is crucial for improving both psychological and physical outcomes. Future research should focus on personalized therapies targeting both stress-related pathways and individualized psychosocial support to optimize patient well-being and treatment efficacy.
