Endoscopic assessment of rectal cancer response in patients undergoing neoadjuvant treatment can predict pathologic complete response when specific criteria are applied. Endoscopic features such as absence of nodularity and ulceration and the presence of a flat, white scar with telangiectasia, if used in conjunction, have a diagnostic accuracy of more than 80% in predicting subsequent pathologic complete response in curative intent surgery. Therefore, it can be utilized for safe omission of surgery in organ-preservation strategies. |
