Core Tip	Colorectal cancer’s lymph node metastasis status impacts prognosis/treatment, but current staging lacks sensitivity. A retrospective cohort (n = 303) study developed a nomogram model using clinical/pathological features, immune cells, CD34, and tumor markers. With area under the curves of 0.856 (training) and 0.899 (validation), and useful decision curve analysis, plus an online tool, it optimizes staging, reduces needless lymph node dissection, and aids neoadjuvant therapy selection.
