Core Tip A significant take away from the study by Aguiar et al is that patients who undergo local repair tend to have notably worse short-term outcomes and overall survival compared to those who opt for gastrectomy. These results collectively indicate that surgical resection, even in urgent situations, may offer survival advantages over conservative repair methods for certain patients. The Aguiar et al study adds relevant data to this complex decision and reinforces the decision that gastrectomy, when feasible, should be performed, as it offers significantly better survival outcomes compared to local repair alone.
