The post hoc analysis of the NEPHRODIAB (nephropathy in diabetes type 2) cohort by Caturano et al challenges the long-held assumption that tighter glycemic control uniformly improves cardiovascular outcomes in type 2 diabetes. Among high-risk patients with microvascular disease, achieving glycated hemoglobin ≤ 7% predicted better cardiovascular survival only under standard care and not only within a structured multifactorial intensive-therapy framework, where other risk factors were tightly managed. These findings suggest that the prognostic value of glycemic control depends on the broader treatment context: Once blood pressure, lipids, and other risks are optimized, further glucose lowering offers limited additional benefit. Thus, individualized targets and risk-factor–based care remains central to diabetes management. |
