Core Tip	Risk factors for carotid plaque in type 2 diabetes mellitus (T2DM) have not been fully elucidated. Shi et al's study identified age, body mass index, fasting plasma glucose, glycated hemoglobin, serum creatinine, urinary albumin-to-creatinine ratio, and serum uric acid as crucial risk factors for carotid atherosclerosis in T2DM patients. However, the study design had certain limitations. To strengthen the validity of the conclusions, a prospective multicenter study incorporating data on diabetes duration and comprehensive medical history is recommended. Furthermore, using machine learning models could facilitate a clearer understanding of causal pathways and enhance cerebrovascular risk stratification in T2DM.
