Core Tip Zhang et al. demonstrate that intestinal obstruction (IO) disrupts the gut-vascular barrier (GVB), evidenced by elevated Plasmalemma vesicle associated protein 1 (PV1) expression, which correlates strongly with systemic infection, organ injury, and poor clinical outcomes. This pivotal study establishes PV1 as a novel biomarker for risk stratification and positions GVB integrity as a critical therapeutic target, redefining IO management from an epithelial barrier-centric model to a GVB-driven paradigm.
