Core Tip In this editorial, we comment on the article by Wang et al, which reports a strikingly high incidence (16.8%-35%) of de novo kidney stone formation within 7 days in patients with acute pancreatitis and hypercalcemia. We argue that this timeline is biologically inconsistent with known lithogenesis. We further propose that the study’s findings do not reflect true “incidence” but rather the “unveiling” of a high prevalence of pre-existing, asymptomatic microlithiasis identified through intensive in-hospital monitoring during a period of metabolic stress. This distinction is critical for accurate interpretation and appropriate clinical management.
