Using simulated data from electronic health records of patients with heart failure with preserved ejection fraction (HFpEF), Mansoor et al found that beta-blockers were associated with lower mortality and fewer heart failure hospitalizations than calcium channel blockers, albeit with more bradycardia and discontinuation. We view this not as a simple “winner-versus-loser” result, but as a signal of underlying HFpEF phenotypes and evolving therapy. Our letter outlines how beta-blockers and calcium channel blockers might be integrated with sodium-glucose cotransporter 2 inhibitors, ARNI, and metabolic agents in phenotype-guided HFpEF care. |
