Core Tip This editorial evaluates a pioneering YOLOv7-based artificial intelligence model for detecting rare rectal neuroendocrine tumors on white-light endoscopy. We highlight the model’s strong performance vs human readers, discuss methodological limitations (static images, class imbalance, permissive intersection over union threshold, lack of enhanced imaging and device heterogeneity), and propose a pragmatic roadmap, improved explainability, multimodal datasets, prospective real-time validation, and regulatory engagement, to translate such artificial intelligence tools safely into clinical practice.
