Core Tip This commentary introduces a precision-psychological framework that reinterprets preoperative anxiety as a stable expression of personality rather than a situational reaction. By viewing emotional stability and conscientiousness as regulatory systems instead of risk markers, it proposes the concept of psychological calibration, aligning temperament and cognition with surgical context. Integrating brief personality-based screening into perioperative care can personalize anxiety management, reduce medication dependence, and strengthen the bridge between emotional science and clinical medicine.
