Core Tip Lin et al’s study demonstrates that ultra-early adjuvant chemotherapy (10-13 days postoperatively) is feasible and safe for advanced gastric cancer (GC) under Enhanced Recovery After Surgery protocols, associated with a hypothesis-generating signal of reduced peritoneal recurrence but no survival advantage. GC’s molecular heterogeneity, inadequate dose optimization, and small sample size limit the study’s conclusions. Precision timing strategies integrating tumor biology (e.g., molecular subtypes), patient recovery status, and dynamic monitoring (e.g., circulating tumor DNA) may offer a pathway to improve outcomes, requiring validation in large, well-designed prospective trials.
