Core Tip This review synthesizes evidence suggesting that serum serotonin (5-hydroxytryptamine) may serve as an accessible biomarker linking levodopa-induced dyskinesia and depression in Parkinson’s disease. Reduced peripheral serotonin levels are associated with both conditions and may reflect shared central serotonergic dysfunction. Yet the relationship between peripheral and central measures -and the biomarker’s predictive specificity-remains unclear. That clarity will require validation through longitudinal, multimodal, and interventional studies. If confirmed, serum serotonin may support early risk identification and guide personalized therapeutic strategies.
