Core Tip	This article demonstrates that the presence or absence of neurosensory foveal tissue is a key determinant of brilliant blue G (BBG)-induced retinal phototoxicity during internal limiting membrane peeling. Surgeons should be especially cautious when operating on eyes with macular holes, as these are more vulnerable to foveal damage due to direct BBG-light interaction. Meticulous control of illumination intensity and dye exposure, along with protective intraoperative strategies, is recommended to minimize the risk of retinal toxicity and optimize visual outcomes.
