Core Tip Artificial intelligence has impact on gastric cancer surgery in several ways. It can improve the risk prediction for perioperative complications as well as planning and monitoring of surgery and treatment using big data. In addition to high prediction accuracy of artificial intelligence systems, they must be interpretable, transparent and applicable for clinical use. Artificial intelligence systems will be very useful for surgery in the future. This article underscores how such clinically transparent artificial intelligence models may support individualized risk stratification and informed postoperative decision-making in gastric cancer surgery.
