Core Tip Postoperative recurrence remains a major challenge in hepatocellular carcinoma management. This study evaluated the complementary diagnostic value of conventional ultrasound (US) and contrast-enhanced magnetic resonance imaging (CE-MRI) in detecting early recurrence after radical hepatectomy. CE-MRI demonstrated superior sensitivity for small and deep lesions and vascular invasion, while US showed advantages in detecting superficial lesions and enabling frequent surveillance. The combined use of US and CE-MRI significantly improved diagnostic accuracy and detection rates. An integrated surveillance strategy using both modalities may enhance early detection and guide timely therapeutic intervention.
