Core Tip This meta-analysis evaluates the prevalence, underlying mechanisms, and clinical impact of cerebral microbleeds in patients undergoing extracorporeal membrane oxygenation. While extracorporeal membrane oxygenation is a life-saving intervention, the intersection of necessary systemic anticoagulation, rapid hemodynamic shifts, and circuit-induced inflammation creates a perfect storm for microvascular brain injury. Our findings highlight cerebral microbleeds as a critical, yet frequently under-recognized, neurological complication. We propose that early neuroimaging and targeted hemodynamic monitoring are essential to mitigate bleeding risks and improve long-term functional outcomes in this highly vulnerable patient population.
