In constrictive pericarditis (CP), the pericardium becomes thickened and fibrotic, compromising diastolic relaxation and causing right-sided heart failure. Clinical diagnosis is often delayed because of overlapping features with commoner conditions like other causes of heart failure and chronic liver disease. In recent decades, non-invasive multimodality imaging has superseded invasive cardiac catheterization as a diagnostic tool. In clinical practice, transthoracic echocardiography is the investigation of first choice. Cardiac magnetic resonance and cardiac computed tomography scan play complimentary roles. At the advent of the 21st century, management of CP is going through a transition – “same therapy for all” to a more customized evidence-based strategy. Anti-inflammatory therapy is indicated in inflammatory phenotypes of CP. For resistant cases, those of non-inflammatory phenotype and the calcified cases, radical pericardiectomy offers the only chance of cure. |
