Core Tip This study demonstrates that more than one-quarter of adults presenting with ketosis have latent autoimmune diabetes in adult, highlighting the risk of misclassification. We identify a practical panel of non-antibody biomarkers, including 2 hours C-peptide, high-density lipoprotein cholesterol, gamma-glutamyl transferase, homeostasis model assessment of insulin resistance, alkaline phosphatase, free triiodothyronine, and fasting plasma glucose, that can effectively differentiate latent autoimmune diabetes in adult from ketosis-prone type 2 diabetes mellitus and predict ketosis onset. These readily available indicators provide a cost-effective diagnostic alternative in settings where autoantibody testing is limited, facilitating early classification and personalized treatment strategies.
