Core Tip The treatment of hypoparathyroidism (hypoPT) after surgery is a major clinical challenge. In this study, a rabbit model of parathyroid ischemia was established by ligating the upper limb vessels of the parathyroid glands. Exosomes derived from umbilical cord mesenchymal stem cells (UCMSC-Exo) were injected into the parathyroid capsule. The results showed that this method could significantly restore parathyroid hormone and serum calcium levels, and alleviate the pathological damage of parathyroid tissue. Mechanism studies revealed that UCMSC-Exo exerted its protective effect by inhibiting the neutrophil cytosolic factor 4-nuclear factor kappa B signaling pathway. This research suggests that UCMSC-Exo may be a promising therapeutic strategy for preventing postoperative hypoPT.
