Core Tip This propensity score-matched study in diabetic patients undergoing pancreaticoduodenectomy shows that adding postoperative octreotide to pancreatic duct stent placement significantly reduces clinically relevant postoperative pancreatic fistula (11.7% vs 23.3%), shortens drain retention and hospital stay, and does not affect overall postoperative glucose metabolism. The combined mechanical and pharmacological approach appears safe and beneficial for the type 2 diabetic patients included in this single-center retrospective cohort.
