Core Tip Autoimmune kidney diseases such as lupus nephritis (LN) and antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis are the major cause of end-stage kidney disease. Kidney transplantation (KT) is an important therapeutic option for these patients. Disease quiescence before transplantation is crucial in patients with LN. Novel biomarkers such as monocyte chemoattractant protein-1, B-cell activating factor, and matrix metalloproteinase-7 help in predicting the post-transplant recurrence. Outcomes may improve with preemptive KT and individualized immunosuppression. In ANCA-associated vasculitis, 12 months of stable remission is recommended, despite the presence of ANCA positivity. The relapse risk is minimized with advanced immunosuppressive regimens, while for post-transplant recurrence, rituximab offers an option. For optimizing long-term graft survival, modifying strategies and emerging cell-based therapies hold promise.
