Rapid advancement in cutting edge basic research in genetics, multi-omics, bioengineering and artificial intelligence has transitioned the field of gastrointestinal pathophysiology from organ-centric models to a systems-biology approach which highlight the complex crosstalk between the gut microbiome, the enteric nervous system, the mucosal immune system, and the enteroendocrine system, producing systemic effects. This narrative review will highlight three primary pathophysiological mechanisms, namely the role of the epithelial barrier integrity, the metabolic influence of the microbiome, and the gut microbiota-distant organ axis in disease pathogenesis. The utility of artificial intelligence and bioengineering models in shaping future therapy will be discussed. |
