Residual stones remain an important and clinically consequential challenge after hepatectomy for hepatolithiasis, yet individualized tools for early postoperative risk stratification remain limited. In this retrospective cohort of 300 consecutive patients, 12 complete prediction pipelines were rigorously compared using repeated nested cross-validation within the development cohort, and least absolute shrinkage and selection operator-penalized logistic regression was selected before evaluation in a strictly held-out internal validation cohort. The final model retained only four routinely available perioperative predictors: Operative time, bilateral stone distribution, T-tube drainage, and postoperative biliary stricture documented before outcome ascertainment, and achieved an internally validated area under the receiver operating characteristic curve of 0.877 with a Brier score of 0.134 and a negative predictive value of 0.944. Its combination of parsimony, interpretability, low data-acquisition burden, and stable internally validated performance represents a practical strength for postoperative risk stratification. The model may help identify patients who warrant intensified surveillance and earlier imaging assessment, while independent external validation remains necessary before routine clinical implementation. |
