Key Words Conventional open liver resection; Laparoscopic liver resection; Primary liver cancer; Prognosis; Liver function
Core Tip Laparoscopic liver resection (LLR) for stage I-II primary liver cancer was associated with less intraoperative blood loss, faster gastrointestinal recovery, better preservation of early liver function, fewer complications (3.33% vs 14.29%), and a higher 6-month recurrence-free survival (70.82% vs 54.08%) than conventional open liver resection. Multivariate Cox analysis identified LLR (hazard ratio = 0.53) and earlier tumor-node-metastasis stage as independent factors associated with improved recurrence-free survival, supporting LLR as an effective minimally invasive option for appropriately selected patients.
