Three APOB variants were identified in a Chinese patient with familial hypobetalipoproteinemia (FHBL)-related cirrhosis who progressed to subsequent hepatocellular carcinoma. FHBL does not have an invariably benign clinical course as the coexistence of compoun heterozygous mutations and metabolic comorbidities, particularly type 2 diabetes, markedly increases the risk of liver disease progression. For patients with cryptogenic cirrhosis and unexpectedly low low-density lipoprotein cholesterol or apolipoprotein B levels, genetic testing is a definitive tool for clarifying the underlying pathogenesis. Regular hepatocellular carcinoma imaging is strongly recommended in the long-term follow-up of FHBL patients with advanced hepatic fibrosis or concurrent metabolic risk factors. |
