This retrospective intention-to-treat study compared primary open Ladd procedure with laparoscopically initiated surgery in 37 children with clinically significant intestinal malrotation. No statistically significant differences were detected in the analyzed perioperative outcomes. Two laparoscopically initiated index procedures required conversion because of unexpected small-bowel perforation or subtotal small-bowel necrosis. The findings support laparoscopic Ladd procedure as a feasible strategy in selected children, provided that conversion is used early when bowel viability or completeness of revision is uncertain. |
