Core Tip Early esophagogastric junction neoplasia with a same-site subepithelial lesion may mimic deep invasion because the surface and contour suggest different depths. White-light endoscopy/magnifying endoscopy with narrow-band imaging may show superficial neoplastic findings, whereas conventional endoscopy shows an overall submucosal tumor-like contour. This study summarizes this rare submucosal tumor-like lesion pattern using 4 institutional cases and 23 published cases. The practical message is that invasion depth should not be judged from contour alone. Integrated white-light endoscopy/magnifying endoscopy with narrow-band imaging, selective endoscopic ultrasonography and en bloc resection when appropriate can improve recognition and histopathologic stratification.
