Core Tip Primary non-response to infliximab (IFX) remains difficult to anticipate before treatment in Crohn’s disease. This multicenter study developed and externally validated an interpretable routine-data model to identify biologic-naïve patients who are likely to achieve week-52 mucosal healing after IFX. Using only baseline clinical and laboratory variables, the model identified a high predicted-response subgroup with a 92.1% mucosal healing rate. These findings suggest that the model may support more individualized pretreatment decisions by recognizing patients highly likely to benefit from IFX.
