| Core Tip |
Gastric cancer prevention in low- and intermediate-incidence settings may require alternatives to uniform population-wide endoscopic screening. Population-based Helicobacter pylori screen-and-treat for primary prevention and risk-based endoscopic screening for the detection of premalignant conditions and early neoplasia are complementary but not equivalent strategies. They may be integrated with clinical risk assessment and locally validated biomarkers within context-adapted pathways reflecting local epidemiology, endoscopic capacity, and quality infrastructure. However, direct evidence that such pathways reduce gastric cancer mortality remains limited. Prospective, setting-specific studies are needed to evaluate their clinical effectiveness, cost-effectiveness, feasibility, participation, and equity. |
