This case demonstrates that resolution of portal venous gas does not mean relief of ischemia. Serial contrast-enhanced computed tomography assessment of bowel wall enhancement - preserved enhancement allowing continued conservative management, intact enhancement confirming restored perfusion - offers a new perspective for guiding conservative treatment in elderly patients with reversible ischemia. Additionally, serial contrast-enhanced computed tomography captured a distinctive migratory edema pattern with progressive venous engorgement, suggesting hemodynamic recovery and favorable prognosis. |
