Core Tip Hepatocellular carcinoma (HCC) progression is governed by dynamic interactions between tumor genomics, stromal elements, hypoxia, and aberrant vascular signaling. The success of immune-antiangiogenic combinations validates the concept that vascular normalization and immune reprogramming are biologically interconnected processes. This review integrates molecular, tumor microenvironment-level, and clinical trial data to define biomarker-aligned, mechanism-based vascular and immune targeting strategies aimed at achieving more durable disease control in advanced HCC.
