Core Tip	Experimental and observational evidence support a biologically plausible gut-kidney axis in chronic kidney disease, but current human trials remain small, heterogeneous, and focused mainly on surrogate biomarkers. This review distinguishes human interventional evidence from animal and mechanistic findings, summarizes inconsistent results for dietary fiber, probiotics, prebiotics, synbiotics, and short-chain fatty acids, and emphasizes reverse causality, safety, and implementation constraints. No microbiota-targeted strategy has established benefit on kidney failure, cardiovascular events, or mortality.
