| Core Tip |
Preservation-fluid selection cannot be separated from donor pathway, temperature control, perfusion, transport, and graft assessment. Direct comparisons among University of Wisconsin, Celsior, and histidine-tryptophan-ketoglutarate are mainly observational, heterogeneous, and confounded; no fluid has established universal superiority. Randomized trials support specific perfusion strategies in defined donation after brain death or donation after circulatory death contexts, but these results should not be extrapolated to isolated fluid choice. Preservation is best interpreted as an integrated pathway that also accounts for access, training, regulation, and cost. |
