Core Tip This randomized trial comparing functional end-to-end (FETE) and overlap esophagojejunostomy, both performed using the self-pulling and latter transected technique (SPLT), in totally laparoscopic total gastrectomy demonstrates that FETE-SPLT is non-inferior to overlap-SPLT with respect to 12-month global health status, with the confidence interval falling within the predefined minimal clinically important difference range. No statistically significant between-group differences were observed in quality-of-life recovery or safety outcomes, but FETE-SPLT required a shorter anastomotic time and fewer stapler cartridges, suggesting an advantage in surgical efficiency.
