Antibody-mediated rejection remains a major cause of long-term kidney allograft dysfunction and graft failure despite advances in immunosuppressive therapy. Antibody-mediated rejection has been attributed primarily to donor-specific antibodies and complement-mediated endothelial injury, but growing evidence indicates that natural killer (NK) cells can also play an important role in mediating allograft injury. NK cells contribute to endothelial damage through antibody-dependent cellular cytotoxicity via FcγRIIIa (CD16) engagement and through antibody-independent mechanisms such as missing-self recognition. Activated NK cells release cytotoxic molecules and pro-inflammatory cytokines that promote microvascular inflammation, glomerulitis, and chronic vascular injury. |
