Core Tip	This minireview presents a formulation-led framework for using metaphors to target cognitive-attentional syndrome processes across psychiatric disorders. Rather than matching metaphors to diagnoses, it links each image to a defined metacognitive therapy target and an immediate intervention, such as attentional disengagement, detached mindfulness, the attention training technique, or intentional non-response. The framework also distinguishes temporary deferral from cautious use in psychotic-spectrum and dissociative presentations and positions metaphors as adjunctive clinical scaffolds rather than independent therapeutic agents.
