Core Tip	This retrospective cohort study of 872 emergency department patients with acute coronary syndrome found that 14.9% developed clinically significant post-traumatic stress disorder (PTSD) symptoms within one month. PTSD symptom positivity was independently associated with more than two-fold higher risk of major adverse cardiovascular events and all-cause mortality over three years. Women, younger patients, and those with ST-segment elevation myocardial infarction were at higher PTSD risk. These findings suggest that screening for PTSD symptoms might be considered for integration into routine post-acute coronary syndrome care to identify high-risk patients, with the potential to improve long-term prognosis.
