Core Tip This report describes the first case of acquired hemophilia A after cholangiocystectomy. Postoperative bile leak with persistent abdominal infection is the key trigger for anti-factor VIII (FVIII) antibody formation. Timely FVIII testing, infection control, immunosuppression, and plasma exchange achieve cure.
