Core Tip	Given the lack of prognostic models specifically for early-onset rectal adenocarcinoma, we developed and validated a nomogram using the Surveillance, Epidemiology, and End Results database and a real-world hospital cohort. Based on eight independent factors, our web-based calculator outperformed the traditional tumor-node-metastasis staging system in predicting cancer-specific survival. This tool offers clinicians a personalized, evidence-based approach to survival assessment and treatment optimization.
