Core Tip	Kidney transplantation in human immunodeficiency virus (HIV) represents a paradigm shift, evolving from being considered an absolute contraindication to a standard of care with growing global acceptance. Advances in antiretroviral therapy, particularly switching potential recipients to integrase inhibitor-based regimens before transplantation, optimized immunosuppressive protocols, and increasing evidence of favorable patient and graft survival have redefined outcomes for HIV positive recipients. Despite challenges such as higher rejection rates, drug-drug interactions, and risks of recurrence, results are comparable to those in HIV negative patients. The use of HIV positive organs in HIV positive recipients has further expanded access to transplantation. This underscores the importance of multidisciplinary collaboration to optimize patient care and ensure long term success.
