Core Tip	Clinicians often view antipsychotic-induced diabetes as a consequence of obesity-linked insulin resistance. Yet, accumulating evidence reveals that clozapine and olanzapine directly impair pancreatic β-cell function via endoplasmic reticulum stress, proinsulin misfolding, and dedifferentiation, allowing diabetes to develop in lean individuals. This review presents an updated paradigm where obesity-dependent insulin resistance and intrinsic β-cell vulnerability co-exist. We highlight the metabolic susceptibility of East Asian populations, discuss how glucagon-like peptide-1 receptor agonists protect cell survival, and outline a precision medicine framework for metabolic risk management.
