Core Tip	We report a case of diffuse large B-cell lymphoma (DLBCL) that developed in the residual ileum 12 years after first onset. A 51-year-old man was diagnosed with primary small bowel DLBCL [germinal center B-cell (GCB) type]. The patient initially underwent an ileal resection and subsequently received six cycles of R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone) chemotherapy for the residual duodenal lesion, achieving a complete remission. At the age of 63, the patient again experienced symptoms of intestinal obstruction. After surgical resection of the ileal lesion, the pathological diagnosis was non-GCB type DLBCL. In both episodes, the lymphoma lesions were observed only in the small intestine.
