Core Tip In adults, a high ammonia level is usually attributed to cirrhosis, yet a substantial minority of patients have normal livers. Sorting these cases by mechanism-excess ammonia production or impaired clearance-turns a broad differential into a focused, stepwise workup. Many causes are reversible, including urease-producing infections, drugs such as valproate, portosystemic shunts, and unmasked urea cycle disorders. Measuring ammonia early in patients with unexplained encephalopathy and normal liver tests, while initiating ammonia-lowering therapy and investigating the underlying cause, can prevent avoidable brain injury.
