This review compares major international guidelines for the risk stratification and surveillance of intraductal papillary mucinous neoplasm, including differences in high-risk and worrisome features, indications for endoscopic ultrasound and surgery, and surveillance strategies. Management largely relies on clinical and imaging features, and how these change over time. Where findings are uncertain or guidelines differ, decisions should consider the combination of risk features, as well as age, comorbidity, surgical fitness, life expectancy, and patient preferences. Cyst fluid and molecular biomarkers can support assessment in selected cases, while the clinical role of newer molecular approaches and artificial intelligence is still being established. |
