Core Tip Reliable tools for predicting myocardial injury among patients with moderately severe acute pancreatitis and severe acute pancreatitis are lacking. Computed tomography/magnetic resonance severity index score and clinical variables including age, blood urea nitrogen, and D-dimer level were identified as independent predictors of myocardial injury. We developed and externally validated a clinical-radiological predictive model integrating these routinely available clinical variables with the computed tomography severity index/magnetic resonance severity index score. The resulting model demonstrated robust predictive performance across independent cohorts. Myocardial injury was independently associated with in-hospital mortality. This model may facilitate timely identification of high-risk patients, closer cardiovascular monitoring, and risk-informed clinical management.
