Core Tip In adults with obesity undergoing gastroscopy, adding low-dose remimazolam to remifentanil increased successful insertion without body movement, coughing, or retching from 63.5% to 81.5% and reduced rescue remifentanil use from 48.1% to 13.0% in the modified intention-to-treat analysis. Sensitivity analyses in all 110 randomized participants showed a consistent direction of effect. Case-report form-recorded desaturation was more frequent with the combined regimen, whereas scheduled four-time-point oxygenation was similar between groups and protocol-defined hypoxemia and mask ventilation were uncommon. These findings suggest improved procedural tolerance while underscoring the importance of close respiratory monitoring.
