Core Tip Comprehensive geriatric assessment can complement tumor staging and routine surgical risk assessment in adults aged 65 years or older with potentially resectable gastric cancer. It identifies functional, nutritional, cognitive, medical, and social vulnerabilities that may be modifiable before or after gastrectomy. Comprehensive geriatric assessment should inform multidisciplinary and shared decisions, not determine treatment through fixed frailty categories. The proposed pathway requires prospective validation.
