Core Tip Human epidermal growth factor receptor 2 (HER2) positivity was independently associated with poor prognosis in gastric signet-ring cell carcinoma. A significant interaction between HER2 status and neoadjuvant chemotherapy (NAC) was observed in the overall cohort. Propensity score-matched analyses showed that the associations between NAC and poorer survival persisted in the HER2-negative and HER2-low subgroups after adjustment for measured treatment-selection factors. However, the association in the HER2-positive subgroup could not be reliably evaluated because only two patients received NAC. These findings should be considered exploratory and warrant prospective validation.
